Sam and Dean Winchester have been through a lot in the years that they spent "saving people, hunting things."
However, after last week's premiere episode, it seems like Sam has finally become aware of their faults as hunters and brothers.

Knowing that his brother will do everything and go through anything to save him, Sam opted not to tell him about the mysterious virus that he was infected with. He chose to sacrifice himself in order for Dean to save baby Amara.
What the brothers didn't know was that the baby had the Mark of Cain all this time. Could she be Darkness that Dean met while he was knocked out?
On the other hand, will Dean figure out that his brother was dying from what could be an infection caused by Darkness?

"Dean (Jensen Ackles) helps Jenna (guest star Laci J. Mailey), the nurse he met after the Darkness fell, get back home safely and then sets off to help Sam (Jared Padalecki) deal with a town infected by a mysterious and deadly gas," the official synopsis reportedly said, according to IB Times. "However, when Jenna falls into dangerous hands, she calls Dean for help and Sam tells him to go back to her. Unfortunately for Sam, he isn't able to handle the townspeople turned monsters as easily as he thought."
Meanwhile, the trailer shows Sam struggling to keep himself sane, Castiel being tortured, Dean finding out about Amara's mark and Crowley dressing up as a priest.
